evaluation of dog for my course?

I'm doing a dog training course and have to write an evaluation form, what things you put in it? I already have a long list, but I figured if there was something good, original ideas for dog lovers. Some examples are: the dog neutered?, Aggressive?, race?, age?, the dog has had previous training? any injury exsisting? The dog was bought from a shelter or pet store? etc. All and all ideas would be brilliant and much appreciated:)

I gather means it has no dogs, or have not been to a training course. It would be helpful to visit a class in session to have a clearer idea of what kind of things have to be evaluated. His example is the dog neutered? " is an indication that you are a beginner and do not know much about dogs. The question should be "the dog has been neutered?" In sterilization, the question is a simple yes / no, but for other questions you may wish to give them more information than a simple yes / no questions will be given. You may want to have a scale of 1 to 5 or 1 to 8 or 1 to 10 for at least part evaluation sheet.
